Emotional Risk Survey

Challenges tend to have an impact on our emotional health which may not always be obvious. This survey is aimed to understand the current state of your emotional wellbeing and offers you important steps you can take to maintain and/or improve your emotional health.

Instructions:
Please read each statement carefully. For each statement, select the option that you feel is best suited to you.
Think about the following statements with regards to how you have been feeling in the past 1 month. Do not think too long about your response, there are no RIGHT or WRONG answers.
